BMI is weight in kilograms divided by height in meters squared (CDC). The CDC classes an adult BMI of 25 to less than 30 as overweight and 30 or greater as obesity (CDC). The labels for semaglutide (Wegovy) and tirzepatide (Zepbound) do not give a BMI number, but their main weight-reduction trials enrolled adults with a BMI of 30 or higher, or 27 to under 30 with at least one weight-related condition. Whether a medication fits is the prescriber's decision.
What do the semaglutide and tirzepatide labels say about weight?
For weight reduction and long-term weight maintenance, the prescribing information for semaglutide (Wegovy) covers adults with obesity, and adults with overweight in the presence of at least one weight-related comorbid condition, along with a reduced-calorie diet and increased physical activity (Wegovy prescribing information, section 1). The tirzepatide (Zepbound) label uses the same wording (Zepbound prescribing information, section 1). Neither label states a BMI cutoff in that section.
The BMI numbers people often quote come from the clinical trials described in the labels. The weight-reduction trials enrolled adults with obesity (BMI of 30 or higher) or with overweight (BMI of 27 to under 30) and at least one weight-related condition, such as treated or untreated abnormal cholesterol or high blood pressure(Wegovy prescribing information, section 14.2) (Zepbound prescribing information, section 14.1). Semaglutide's label also covers children 12 and older with obesity, which is judged by BMI percentiles for age and sex rather than the adult categories (Wegovy prescribing information, section 8.4).
